How do I address a married couple where the female is a doctor?
As we know from Etiquette 102, the woman’s name always goes first. So, even though a couple is married – and they might even have the same last name – she is addressed first.* Also, please note that is is common to use ‘Dr.’ (abbreviated). Using ‘Doctor’ (spelled out) is usually reserved for the most formal of occasions: black or white tie. Though using ‘Dr.’ or ‘Doctor’ is a personal decision.

How do I address a married couple that are both doctors with the same last name?
The Doctors Lewis
If the man is a doctor, you would address the envelope the same as a standard envelope:
Dr. and Mrs. Thomas Stoneking
